This form is for online registration and payments only.
Unity of Dayton has a limited number of 6 foot tables no extra charge. (Available on a first come, first serve basis.) Do you wish to have a table provide?
Chairs will be provided. How many will be needed?
- Be sure to have a covering for your table and all boxes stored under the table.
- Set up begins at 8:00am. Closing time is 5pm. DO NOT break down your table before 5pm.
- Unity of Dayton is a NO SMOKING campus.
- Make check payable to: Unity of Dayton. Credit cards and PayPal accepted.
- Unity of Dayton shall not be held responsible for any liability, lost, stolen, damaged merchandise or personal property, or any physical injury incurred during the bazaar including setup and take down times.
- NO REFUNDS AFTER October 1, 2023
- Payment is due at time of reservation – clicking the button below will take you to PayPal where you can pay. If we do not receive payment, your reservation will not be valid and will be discarded.
